Summary 13;
The Community Health Worker (CHW) will be responsible for helping and supporting children and their families to acquire preventive and follow:up health care needs, navigate and access healthcare services, educate families on the importance of healthcare follow:ups, reinforce disease specific education, perform medication reconciliation, and provide services to address barriers to healthcare follow:up. The CHW works collaboratively with a team of healthcare professionals to provide patients with opportunities to stabilize and improve their health and will work specifically with the Memphis Community HIV Network. Models appropriate behavior as exemplified in MLH Mission, Vision and Values.

REQUIRED:
High School Education or equivalent and three years of experience working in a position requiring interaction with customers or patients.

Current drivers license and automobile for daily use in the position is required. Must maintain auto liability insurance.

PREFERRED:
Bachelors degree in Health, Social Services, Human Services or related field.
Prior experience working in community outreach preferred. Individuals with personal or professional experiences with HIV and/or AIDs, or with the Memphis HIV community, are encouraged .

:Establishes trusting relationships with patients and their families while providing general support and encouragement.
:Assesses and addresses the strengths of families and the barriers to obtaining preventive and follow:up health care.
:Collaborates with family to develop and implement a plan to access preventive and follow:up health care.
:Coordinates with community health care providers, including primary care physicians, other out:patient services, and Methodist Le Bonheur Childrens Hospital.
:Maintains and documents accurate data related to the program.
:Performs other duties as assigned. 13;
Physical Requirements 13;
The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
Must have good balance and coordination.
The physical requirements of this position are: light work : exerting up to 25 lbs. of force occasionally and/or up to 10 lbs. of force frequently.
The Associate is required to have close visual acuity to perform an activity, such as preparing and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading.
